In order for you to start borrowing any library materials and accessing your account in Koha (Library's digital catalog), you will need to fill out this form. This form must be filled out and signed acknowledging that you will be held responsible for any borrowed library items that become lost, damaged, or late under your name.

All borrowers are responsible for all library materials charged to their ID.

It is the borrower’s responsibility to:

• Update phone, email, or other contact info by contacting Library Staff or editing the info under “Your Account” in Koha
• Know when materials are due by checking the Due Date sticker on the item or reviewing the transaction email receipt of the date due for each item or checking their account summary in Koha
• Return materials promptly
• Care in handling all library materials
• Contact Library or use Koha to renew items if they cannot return it on time

Students
Students will need to pay for any late, lost, or damaged fees. If the overdue item has not been returned for more than 30 days, it will be marked “LOST” and the student will have a hold on their account and incur $10 late fee per overdue item. After 60 days overdue, the student will need to pay the replacement cost per LOST item. Any lost fees incurred must be paid before graduation.

Staff & Faculty
Staff & Faculty may be excused of any late, lost, or damaged fees, but will need to return materials promptly.
